29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s): a) On or about January 31, 2024 and at times prior, MD Roofing, as the controlling employer, did not ensure that subcontractor employees exposed to fall hazards of 9 feet 6 inches to the ground below, while performing roofing operations on a residential house utilized fall protection. Subcontractor employees were also not wearing eye protection while using a pneumatic nail gun to secure shingles to the residential roof. The portable ladder that subcontractor employees were using to access and egress the roof did not extend 3 feet past the desired landing.
