29 CFR 1926.753(c)(1)(i): Cranes being used in steel erection activities must be visually inspected prior to each shift by a competent person; the inspection shall include observation for deficiencies during operation: On or about February 01, 2024, employees engaged in commercial construction activities, operating Demag Crane (AT360-135 / Model: AC-435) were exposed to crush-by and struck-by hazards, due to cotter pins missing from the U-bolts on outriggers.

On or about February 01, 2024, employees engaged in commercial construction activities, operating Demag Crane (AT360-135 / Model: AC-435) were exposed to crush-by and struck-by hazards, due to the operator cab window having a significant crack that hampered the operator's view.
