29 CFR  1926.403(e): All splices and joints and free ends of conductors were not covered with insulation equivalent to that of the conductors or with an insulating device designed for the purpose.  Location: 2315 East 5th Street, Brooklyn, NY.  On or about 1/31/2024,  a) The free end of the conductors of a temporary lighting strand was wrapped with white tape.  The taping did not have equivalent insulation and protection as the original insulation and the outer jacket of the cord.

The outer jacket of the cord was not connected to the body of the plug.

Location: 2315 East 5th Street, Brooklyn, NY.  On or about 1/20/2024,  a) Employees worked from scaffold platforms that were not fully planked.  The spacing between the edge of the platform and the uprights were more than 3 inches.
