29 CFR  1910.26(e): The employer did not ensure that portable dockboards are equipped with handholds or other means to permit safe handling of dockboards.  a) Loading Docks, 555 S. BB King Blvd, Memphis, TN:  On or about 01/31/2024, and at times prior, the employer exposed employees to crushing injuries and death, in that dockboards with broken safety locks and gas shocks, which could result in the unintended and uncontrolled deployment of the dockboards, were used for loading and unloading mail using PITs.  b) Loading Docks, 555 S. BB King Blvd, Memphis, TN: On or about 01/31/2024, and at times prior, the employer exposed employees to caught in and crushing hazards, in that dockboards with missing handles were used for loading and unloading mail which required employees to lift and lower them by hand.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) Re-wrap cage, 555 S. BB King Blvd, Memphis, TN: On or about 01/31/2024, and at times prior, the employer exposed employees to fire related injuries, in that, the fire extinguisher mounted to the re-wrap cage was blocked by combustible materials, delaying access to the extinguisher and possibly exposing employees to combustible materials during a fire in order to reach the extinguisher.

29 CFR  1910.176(a): Where mechanical handling equipment was used, sufficient safe clearances were not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made.  Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked.  (a) Mail processing area, 555 S. BB King Blvd, Memphis, TN: On or about 01/31/2024, and at times prior, the employer exposed employees to struck-by hazards, in that the aisles were blocked by pallets of cardboard, APC carts, stacks of plastic pallets, and other materials being stored on the floor.  (b) Mail processing area, 555 S. BB King Blvd, Memphis, TN: On or about 01/31/2024, and at times prior, the employer exposed employees to struck-by hazards, in that aisles used for both pedestrian traffic and PIT traffic were not clearly marked forcing pedestrians and PITs to work in close proximity with no protective barrier or separately marked aisles.

29 CFR 1904.29(b)(2): The employer did not fill out or correctly fill out an OSHA Form 301 or equivalent for each recordable injury or illness.  a)  On or about 01/31/2024, and at times prior, the employer failed to fully complete the OSHA Form 301 Injury and Illness Incident Reports for injuries and/or illnesses included in their 2023 and 2024 calendar year OSHA 300 Logs on the following:  Case No. 2417-64, Case No. 2363-50, Case No. 2410-67, Case No. 2618-91, Case No. 2629-73.  The was no information provided of what the employee was doing before the incident,   incomplete description, object or substance involved was not identified, and the 301 was not certified by a company official.
