29 CFR  1910.22(a)(3): Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a) On 4/3/2024 the employer did not ensure that the walking-working surface of equipment such as but not limited to; 526 Milacron Injection Molding Press (model # MG1800-607) were kept free from slippery conditions while employee was required to climb up the press to change out the mold, exposing employees to a fall hazard of approx. 9 ft.

29 CFR  1910.22(c): Access and egress.  The employer must provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  a) On and before 2/1/2024 the employer did not provide a safe access to the top of equipment such as but not limited to the Milacron #526 Press (model # MG1800-607) in that employees were exposed to fall hazards while transitioning from the floor to the top of the presses by climbing on the presses in order to attach water lines and unhook hoisting chains.

29 CFR  1910.28(b)(6)(i): (i) Each employee less than 4 feet (1.2 m) above dangerous equipment is protected from falling into or onto the dangerous equipment by a guardrail system or a travel restraint system, unless the equipment is covered or guarded to eliminate the hazard.   a) On or before 2/1/2024 the employer did not require the use of fall protection system for employees who were exposed to the fall hazards of approximately 9' while performing such tasks as mold changes. During the mold changing process, employees would climb on top of equipment such as but not limited to the; Krauss Maffei molding press (Model # MX1600-55000) while hooking and unhooking chains connected to the molds.   b) On or before 2/1/2024 the employer did not require the use of fall protection system for employees who were exposed to the fall hazards of approximately 9' while performing such tasks as mold changes. During the mold changing process, employees would climb on top of equipment such as but not limited to the; Milacron molding press (Model # MG1500-413) while hooking and unhooking chains connected to the molds.  c) On or before 2/1/2024 the employer did not require the use of fall protection system for employees who were exposed to the fall hazards of approximately 9' while performing such tasks as mold changes. During the mold changing process, employees would climb on top of equipment such as but not limited to the; Uniloy-Milacron molding press (Model # SF500 WP646) while hooking and unhooking chains connected to the molds.

29 CFR  1910.30(a)(1): Fall hazards.  Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  a) On or before 2/1/2024 the employer did not provide fall hazard training that would ensure that each employee working at heights of approximately 9' were protected from a fall hazard by a fall protection system, prior to performing mold changes to the 26 injection molding machines located through-out the facility.  b) On 4/3/24, the employer did not ensure that fall hazard training provided prior to employees using the fall protection system above the ? mold machine was adequate so that the employees understanding the factors of total fall distance and how to properly utilize the fall protection system to ensure that the cable was not greasy, what the total fall distance would be if the cable was not directly over the employees, the difference between a restraint system and a fall protection system.
