29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge that was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about January 31, 2024, the employees performed repair work on a second story section of the a roof that was greater than six feet above the lower level without fall protection.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about January 31, 2024, employees utilized a Werner Twenty-four (24) foot extension ladder to access the work area. The ladder was not extended three (3) feet above the landing or secured.
