29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation:  a) On or about January 29, 2024, the employer did not provide authorized lockout/tagout training to protect an employee performing servicing and maintenance on the Baker Band Resaw Model BBR-O"ABX", including inspection of the conveyor.  b) On or about February 1st, 2024, the employer did not provide lockout/tagout training to protect an employee performing servicing and maintenance on the ASCO P281 Dry Ice Pelletizer.

29 CFR  1910.1200(e)(1):  The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about February 1, 2024, where hazardous chemicals such as solid dry ice and gaseous carbon dioxide are present, the employer failed to develop and implement a hazard communication program that described how the labeling, safety data sheets, employee information and training, and other requirements of this section will be met.

29 CFR  1910.1200(h)(3)(i):  Employees were not provided effective information and training on the methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area (such as monitoring conducted by the employer, continuous monitoring devices, visual appearance or odor of hazardous chemicals when being released, etc.):   On or about February 1, 2024, where hazardous chemicals such as solid dry ice and gaseous carbon dioxide were present the employer failed to provide hazard communication information and training on the methods and observations used to detect hazardous chemicals in the work area. Employees on the dry ice production floor were not trained on the function and meaning of the carbon dioxide monitors, nor on what the readout meant and at what concentrations to take actions to protect themselves.
