On or about 2/01/2024 and thereafter, employee(s) performed routine, integral service and/or maintenance on #2 Roll Laminator, such as manual scraping / cleaning roller(s). without shutting off and locking out power to the unit, or using alternative effective protective means such as incorporating a "jog" or "inch-safe" mode on the machine drive controller; thus exposing employee(s) to hazard of having hands / fingers crushed by ingoing nip-point(s) of the rollers.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) 322 Cranbury Half Acre Rd. East Windsor, NJ 08512. On or about 2/1/2024 and thereafter, employee(s) performed service and maintenance on production equipment such as the #2 Roll Laminator, which included scraping off glue from roller(s) and clearing paper jams, that involved placing parts of their body into danger zone(s) of operating machinery. Periodic audits / inspections were not performed by the employer to ensure compliance with provisions of the Lockout/Tagout Standard (29 CFR 1910.147).
