(a) On January 29, 2024, at 316 Flint Drive in Bismarck, North Dakota; employees who were working from an elevated forklift work platform were exposed to fall hazards due to the platform missing a complete guardrail system and no other fall protection systems in place. The exposed employee had not been trained in the detection and avoidance of hazards associated with working from scaffolds, nor of the proper use and limitations of the scaffold.
