29 CFR  1910.215(a)(4): On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   (a)  Building 3 - On or about February 1, 2024 the employer did not have the tool rest guard adjusted to one eighth inch from the grinding wheels on a dual head 6-inch bench grinder exposing the employee to a struck-by hazard.
