29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) At 4400 North Park Dr, Opelika, AL; On January 26, 2024, and at times prior employees were exposed to crush-by hazards when removing debris from the NHP-200 Hydraulic Press S/N 1787 without energy control procedures being developed, documented, and/or utilized to control potentially hazardous energy. b) At 4400 North Park Dr, Opelika, AL; On January 30, 2024, and at times prior employees were exposed to struck-by hazards when performing cleaning activities on the molds of the PSM machine without energy control procedures being developed, documented, and/or utilized to control potentially hazardous energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation. a) At 4400 North Park Dr, Opelika, AL; On January 30, 2024, and at times prior thereto, employees were exposed to crush-by hazards when performing cleaning activities, such as but not limited to, removing debris from the lower die on the NHP-200 Hydraulic Press S/N 1787, in that each employee had not been trained on lockout/tagout in accordance with the standard. b) At 4400 North Park Dr, Opelika, AL; On January 30, 2024, and at times prior thereto, employees were exposed to struck-by hazards when performing cleaning activities on the molds for the PSM Skin Machine, in that each employee had not been trained on lockout/tagout in accordance with the standard.
