Santa Rosa Beach, FL. 32459: On or about February 1, 2024, and at times prior to, the employer permitted employees to work from a job-made work platform that was being elevated by a SkyTrak Forklift Model # 10054, which was not approved for that use by the manufacturer, exposing employees to fall hazards of up to 21 feet.
