29 CFR 1910.141(a)(5): Enclosed workplaces were not so constructed, equipped, and maintained as to prevent the entrance or harborage of rodents, insects, or other vermin:  On or about January 26, 2024, employees performed work duties in and around areas such as, but not limited to the stockroom and storage/break room with a rat infestation to include live and trapped rats.  The employer did not have an effective means to prevent rats from entering.

29 CFR 1910.157(c)(1):  Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  On or about January 26, 2024, in the old office/break room, access to the portable fire extinguisher was impeded by a chair and a box of American flags.
