29 CFR 1910.110(f)(2)(i): Containers in storage were not located so as to minimize exposure to temperature rise, physical damage, or tampering from unauthorized persons: On or about February 2, 2024, at the cross dock located at 72 Weldon Parkway in Maryland Heights, Missouri. Two tanks of liquefied petroleum gas, not in use, were not stored to minimize physical damage. The tanks were located outside the storage cabinet and located in the travel path of employees and the Hyster 50 powered industrial truck. Employees were exposed to fire and explosion hazards.

29 CFR 1910.178(p)(1): Powered industrial truck(s) with defect(s) or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s): On or about February 2, 2024, at the warehouse cross dock located at 72 Weldon Parkway in Maryland Heights, Missouri. The Hyster forklift, m/n S50FT, s/n F187V10303E, was operated with the liquefied petroleum gas tank alignment pin missing. The Operating manual clearly addresses the significance of inspecting the fuel system and components before use. Employees were exposed to fire and explosion hazards while operating the powered industrial truck without the pin securing the liquified petroleum gas tank.

Exit routes were obstructed by boxed and unboxed products, wheeled carts, and other debris. This impeded the direct means of egress for employees in the event of an emergency.

Fire extinguishers in the cross-dock area were not mounted and readily accessible for employee use in the event of an emergency. An extinguisher was located on the floor adjacent to the North Bay door and three extinguishers were located on a table near the liquified petroleum gas storage cabinet.
