29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. At a worksite located at 1319 Vine Street, Kansas City, MO, an employee was exposed to fall hazards. The employer exposed the employee to these hazards by allowing them to perform construction work on the roof of a structure at heights greater than 6 feet above the next lower level without the use of fall protection or mitigation measures.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a worksite located at 1319 Vine Street, Kansas City, MO, an employee was exposed to fall hazards. The employer exposed the employee to fall hazards by allowing them to use a portable extension ladder while the ladder's side rails did not extend at least 3 feet above the level for which the ladder was being used to gain access.
