29 CFR  1910.37(b)(2): Each exit was not clearly visible and marked by a sign reading "Exit":   On or about February 5, 2024, and times prior thereto, the employer did not ensure the exit doors were equipped with a serviceable EXIT sign exposing employees to entrapment hazards.  a) In the main entrance, no EXIT sign was placed above the door, b) In building number 1 west door, no EXIT sign above the door, c)  In building number 1 south door, no EXIT sign above the door, d)  In building number 2 , north side no EXIT sign above the door.

29 CFR  1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel.  On or about February 5, 2024, and times prior thereto, north side of the tool room, the employer did not ensure adjustable tongue guards were installed prior to employees operating the bench grinder, exposing them to struck-by flying objects.

29 CFR  1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.  On or about February 5, 2024, and times prior thereto, extension cords, zip tied and wrapped around metal conduit, was used as permanent wiring to energize the machines, exposing employees to electrical shock hazards.  a) The mistaway located on top of the Infinity machine was plugged into an extension cord wrapped and zip tied to conduit, b) At the Fadal machine an extension cord zipped tied and wrapped around conduit was used to energize a fan and computer.
