29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by struck by hazards: At the workplace located at 17501 W 98th St #14-39 Lenexa, Kansas, employees operating machine #3 (SHM 2) were exposed to struck-by hazards due to the machine's hydraulic arms using ratchet straps to hold the paper rolls from accidentally releasing.

29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy: At the workplace located at 17501 W 98th St #14-39 Lenexa, Kansas, employees engaged in processing paper in the bailer room were exposed to electrical burn hazards due to no protective cover being provided for the electrical junction box on the south side of the bailer.
