On or about 02/03/2024, at 33625 7902 Citrus Park Town Center Mall, Tampa, FL 33625, an employee was working/walking atop a zip-line/ropes-course at an indoor amusement park, wherein the ropes-course platform height was approximately sixteen feet above the ground, and during which time employee had not attached his lifeline to his body harness; therefore exposing the employee to a fall hazard.
