This condition exposed employees to shock, electrocution, and fire hazards. (b)Innovative Office Solutions LLC at 711 West Russell Street, Sioux Falls, SD 57104: On or about and at times prior to February 6, 2024, the employer did not ensure flexible extension cords were properly utilized in that extension cords were attached to building surfaces and machine surfaces via zip-ties to create a permanent wiring configuration for portable electronic devices to be charged or used in "Room 2". This condition exposed employees to shock, electrocution, and fire hazards.
