29 CFR 1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) Jobsite: 906-16 N. Broad St. Philadelphia, PA 19130 - On or about August 15, 2023, the employer did not ensure that employees performing manual demolition work from the roof of a single story building, exposed to approximately 1 feet by 5 feet hole were protected from stepping into or falling through the hole approximately 10 feet to the lower level by the use of personal fall arrest systems, a cover, or a guardrail system erected around the hole.
