29 CFR 1926.1053(b)(16): Portable ladder(s) with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not removed from service: On or about 2/6/2024, at 5222 Nevola Avenue, Ave Maria, Florida, four employees were exposed to a 10-foot fall hazard while using a 20-foot extendable ladder that was missing approximately 6 inches of one of its siderails (i.e. from the cleat to the first rung at the lower end) and its right locking device was inoperable.
