29 CFR 1910.145(f)(5): Danger tags were not used where an immediate hazard presented a threat of death or serious injury to employees: a) Jobsite, 4201 Grand Avenue, Pittsburgh, PA 15225: On and before January 22, 2024, the area adjacent to the outfeed conveyor on a Smetco PS40-3 gang stacker was not marked with Danger tags to warn employees of struck-by hazards created by the periodic collapse of pallet stacks as they exit the stacker machine.
