29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows: (i) as part of a complete personal fall arrest system which maintains a safety factor of of at least two; and (ii) under the supervisor of a qualified person: On or about February 6, 2024, at a worksite located at 604 S. Lincoln Street, Elkhart Lake, Wisconsin at a new construction site; employees utilizing personal fall arrest systems with anchor points affixed to a concrete floor, which was not permitted by the original equipment manufacturer, exposing employees to a fall hazard over six feet.

29 CFR 1926.503(a)(2)(ii): The employer shall assure that each employee has been trained, as necessary, by a competent person qualified in the following areas: The correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used; On or about February 6, 2024, at a worksite located at 604 S. Lincoln Street, Elkhart Lake, Wisconsin at a new construction site; the employer did not ensure that employees were trained by a competent person in the correct procedures for erecting, maintaining, and inspecting the fall protection system(s) to be used in that the employees did not know the correct procedures for installing and inspecting the Werner Reusable Roof Anchors used in the fall protection systems.
