29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel. On or about February 6, 2024, and at times prior thereto, in the east bay area, the employer did not ensure adjustable tongue guards were installed prior to employees operating the table grinder, exposing them to struck-by flying objects.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinders were designed to accept caps, were not always in place, hand-tight, except when cylinders were in use or connected for use. On or about February 6, 2024, and at times prior thereto, unused compressed air cylinders were stored without the protective cap exposing employees to struck-by hazards. a) South bay area, one oxygen compressed cylinder was stored without the safety cap, b) South bay area, one Acetylene cylinder was stored without the safety cap.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screw. On or about February 6, 2024, and at times prior thereto, in the west bay area, the employer's Eaton General Duty switch box power cord had the outer sheathing pulled away from the terminal exposing the internal wiring exposing employees to electrical shock hazards.
