29 CFR  1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered:  a. On or about February 8, 2024, at the above address, employees were required to operate Komatsu powered industrial vehicles equipped with front end attachments that were not provided with a secondary load plate decal that identifies use with the eighty inch fork extensions. Employees were thereby exposed to injuries associated with struck-by hazards.
