29 CFR 1926.404(b)(1)(iii)(C):The employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Location: Worksite On or about February 8 2024, and at times prior, cord sets were available for use where the cord caps had been replaced and the employer had not ensured that the cord's original rating had been restored thereby exposing employees to electrical shock hazards.
