OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards: a) Axis Energy Services LLC at East Lookout Butte Unit, Well #24C-14NH Baker, MT 59313: On or about February 5, 2024 and at times prior, employees were exposed to struck by hazards while performing hydrotesting operations. The rig crew was using a 6 line string up, double fast line operation with approximately 54,195lbs in the well bore, resulting in excessive loading on the drawworks. b) Axis Energy Services LLC at East Lookout Butte Unit, Well #24C-14NH Baker, MT 59313: On or about February 5, 2024 and at times prior, employees were exposed to struck by hazards while performing hydrotesting operations without the use of a safety device which is designed to prevent the traveling block from striking the crown block. Among other methods, one feasible and acceptable method of abatement to correct this hazard would be for the employer to follow the manufacturers' recommendations that state if using double fast line operation, do not pull over 30,000 pounds (13,600kg) with 4-line string up or 40,000 pounds (18, 150kg) with 6-line string up to avoid excessive loading on drawworks. Follow API 54 2019 with 2021 Addendum 6.10.2 Drilling and well servicing equipment shall be set up and checked for proper installation prior to commencing work in accordance with company guidelines. Utilize the employer installed equipment, Posi-stop device, to protect the crown and floor of the Rig while working.
