On or about: February 7, 2024 Location: 140 Samsondale Ave., West Haverstraw, NY 10993. Eastern/left section of the air recirculation chamber. a) The top guardrail of the scaffold in use by a worker was measured between approximately 36-37 inches.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about: February 7, 2024 Location: 140 Samsondale Ave., West Haverstraw, NY 10993. Eastern/left section of the air recirculation chamber. a) Scaffold in the eastern/right section of the air recirculation chamber was not properly inspected by a competent person for defects. Top guardrail did not meet the appropriate height per OSHA standards.
