Location: Back side of the building @ 503 Mount Pleasant St. New Bedford, MA On or about 02/09/24 scaffold platform was not fully planked where employees were working from the ladder jack scaffold.

29 CFR 1926.451(g)(1):29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section: Location: Back side of the building @ 503 Mount Pleasant St on the side of Auto Zone, New Bedford, MA On or about 02/09/24 employees were exposed to falls of 16 feet to the ground below, where a guardrail system or, personal fall arrest system was not in use.
