(a): Exterior Scaffold - One employee was exposed to fall hazards of approximately 15 feet while working on a pump-jack scaffold that was not properly secured. The employee was not utilizing a personal fall arrest system and the pump jack scaffold was not equipped with a proper guardrail system, on or about February 1, 2024.
