29 CFR  1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets:   Worksite located at 7051 Pacific St., Kansas City, Missouri- Employee was exposed to an overhead hazards  in that the employee was observed working and walking underneath an employee who installing siding on pumpjack scaffold and the employee on the ground was not wearing a hard hat.

29 CFR  1926.451(g)(4)(i):Guardrail systems were not installed along all open sides and ends of platforms:    Worksite located at 7051 Pacific St., Kansas City, Missouri- Employee were exposed to fall hazards and were not protected by falling by the use of guardrails or personal fall arrest systems. The pumpjack was missing the end-rails as well as a midrails underneath the work platform.
