On or about February 6, 2024, during roofing operations on a residential structure located at 1689 E. Sapphire Dr. Hudson, OH 44236; the employer exposed employees to a fall hazard by not ensuring the side rails of the portable ladder being used for roof access, extended at least 3 feet above the upper landing surface.
