At the jobsite, employees were not wearing hardhats while working below other employees that were roofing a church, exposing employees to struck-by hazards from falling objects.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At the jobsite, employees operating a Dewalt nail gun to perform roofing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.

29 CFR  1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.    At the jobsite, employees installing roofing shingles were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access;    At the jobsite, Employees engaged in installing roofing material, were exposed to fall hazards while using a ladder that did not extend at least three feet above the roof's landing.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing roofing activities were not provided fall protection.  The employer did not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
