On or about February 2, 2024  the employer failed to provide and install a fall protection system before employees began working on the installation of new balcony patios. As a result employees were exposed to a fall hazard of 12'3" above ground level.  The employer Bonilla Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.502 (a)(2), which was contained in OSHA Inspection Number 1664880, citation number 1, item number 2,and was affirmed as a final order on 8/14/2023, with respect to a workplace located at 7832 Five Mile Rd, Vantage at Anderson Towne Center, CINCINNATI, OH 45255.
