29 CFR 1910.178(n)(7)(i): When ascending or descending grades in excess of 10 percent, loaded trucks were not driven with the load upgrade. (a) Facility, 20 Museum Road, Washington, PA: On or about February 8th, 2024, the operator of a powered industrial truck (Hangcha forklift) was traveling with the load downgrade at a slope of over 10 percent. The load (2 spools, approximately 5,000 pounds each), rolled off of the forks of the powered industrial truck and severely injured the powered industrial truck operator who got off of the powered industrial truck.

The two spools rolled off the forks of the powered industrial truck and severely injured the operator.
