29 CFR  1926.502(d)(17):The attachment point of the body belt shall be  located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  On or about February 6, 2024, the employer did not ensure that employees connected their harnesses to fall protection anchorage points correctly.  Employees performing work at heights in excess of 20 feet had their lanyards attached to the front side of their harnesses, exposing them to serious injury in the event of a fall.  Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
