29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: On or about February 8, 2024, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye and face hazards resulting from the release of liquid chemicals. Employees working within the Sierra 750TONSBL (S/n:126004) machine were exposed to hydraulic fluid while performing maintenance on a hydraulic cylinder. The hydraulic cylinder failed and saturated employees with pressurized hydraulic fluid absorbed dermally by the employees. Failure to ensure employees use appropriate eye and face protection while servicing hydraulic cylinders exposed employees to potential tissue necrosis, internal bleeding, and blindness.

29 CFR 1910.138(a):The employer did not select and require employees to use appropriate hand protection when employees hands were exposed to hazardous condition where necessary: On or about February 8, 2024, the employer did not ensure that each affected employee used appropriate hand protection when exposed to hazards resulting from the release of liquid chemicals. Employees working within the Sierra 750TONSBL (S/n:126004) machine were exposed to hydraulic fluid while performing maintenance on a hydraulic cylinder. The hydraulic cylinder failed and saturated employees with pressurized hydraulic fluid absorbed dermally by the employees. Failure to ensure employees use appropriate hand protection while servicing hydraulic cylinders exposed employees to potential tissue necrosis, internal bleeding, and other adverse health effects.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): On or about February 8, 2024, the employer did not ensure that authorized employees followed established procedures for the application of energy control for the Sierra 750TONSBL (S/n:126004) recycling machine. An employee performing maintenance activities on the hydraulic cylinder located within the point of operation of the Sierra 750TONSBL machine was not protected from the release of hazardous energy. Failure to ensure authorized employees followed established procedures for the control of hazardous energy exposed employees to amputation hazards, laceration hazards, and potential death from the pressurized hydraulic system.
