29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a) At or near 127 South St, Passaic NJ - Ceiling 4th Floor: Employees without any means of fall protection were exposed to fall hazards of approximately 40 feet, while engaged in framing activities, on or about  2/12/2024.

29 CFR  1926.1052(b)(1): Where the treads and/or landings of pan stairs were to be filled in with concrete or other material at a later date, foot traffic was not prohibited on the stairway:   a) At or near 127 South St, Passaic NJ  -  Interior North Stairway - 4 levels: Employees were exposed to fall hazards of approximately 15 feet while engaged in different construction activities utilizing a steel stairway that had not been filled with concrete nor wood on all 4 levels, on or about 2/12/24.
