29 CFR 1926.502(d)(6)(i): Snaphooks were engaged directly to webbing, rope or wire rope: a) On or about February 14, 2024, employees were exposed to fall hazards while installing a skylight and using a Palmer lifeline 6-foot shock absorbing lanyard (product code V503201ARG serial no. 108450) which was wrapped around a beam and tied back to itself. The lanyard was not equipped with a snaphook that was designed for tie-back according to the product user manual.
