29 CFR  1910.101(b):The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:  a) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance shop, inside the Maintenance Closet: a compressed gas cylinder, unknown content, was not secured from being knocked over, exposing employees to fire and explosion hazards.  b) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance shop, a flammable gas cylinder with the sticker damaged, was not secured from being knocked over, exposing employees to fire and explosion hazards.  c) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance shop, the labels applied by the gas manufacturer to identify the contents of two compressed gas cylinders were defaced or removed.   NOTE: Certification and documentation of abatement must be submitted for all instances of this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR  1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:  a) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance shop: a fire extinguisher, ABC dry chemical, model TGP 10G was obstructed by broomsticks and paint sticks, exposing employees to fire hazards.   b) On or about February 14, 2024, at the Mafolie Hotel and Restaurant, in the hallway between hotel rooms 106 and 107: a fire extinguisher, ABC, dry chemical, 10 pounds, model A055, serial number B14697080, was obstructed by a large plant, exposing employees to fire hazards.  NOTE: Certification and documentation of abatement must be submitted for all instances of this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR  1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  a) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance shop: an electrical panel, 240 volts, that control power to the air conditioning system and lights at the bar, was obstructed by a small refrigerator and boxes, exposing employees to electrical hazards.  NOTE 1:  Mafolie Hotel and Restaurant was previously cited for a violation of this occupational safety and health standard, 29 CFR  1910.303(g)(1), which was contained in OSHA inspection number 1413232, citation number (1) item number (1) and was affirmed as a final order on 04-10-2020, with respect to this workplace.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.

29 CFR  1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) On or about February 14, 2024, at the Mafolie Hotel and Restaurant Maintenance Shop: the employer did not implement their written hazard communication program to ensure adequate labeling of chemicals. A chemical detergent, brand 'Roma', was not placed in a container with a label to identify it, exposing employee to chemical hazards.  NOTE: Certification and documentation of abatement must be submitted for this citation item as required by CFR 1903.19(c) and (d), respectively.
