29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following: On or about February 15, 2024, the employer did not develop and maintain a complete hazard communication program, exposing employees to Silica and Portland cement. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. On or about February 15, 2024, employees performing masonry operations were not protected from a fall while working from a metal frame scaffold, exposing the employee to a fall of 15.5 feet. ROLANDAS VERKYS dba SUBURBAN MASONRY INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1), which was contained in OSHA inspection number 1530877, citation number 1, item number 2 and was affirmed as a final order on November 4, 2021, with respect to a workplace located at 22919 Perth Ct. Frankfort, IL 60423. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
