29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about February 8, 2024, employees were exposed to a fall hazard of 10 feet while completing the removal of shingles from the roof of a residential home located at 947 Bell Road, CHAGRIN FALLS, OH 44022. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 03 feet above the upper landing surface.
