This condition exposed employees to struck-by and caught-between hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: On or about February 15, 2024, employees were exposed to electrical shock and burn hazards in the following instances: a) Production Line #13: The Melt Pump Control Box to the extruder had a strain relief that was broken and electrical wires were exposed. b) Production Line #4: The control panel to Grinder #34 had a strain relief that was broken and wires were exposed.
