29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: Welding Shop: On/before February 12, 2024, in the following instances, two Class A fire extinguishers were not properly staged. Employees were exposed to smoke and burn related injuries from the potential fire hazard. a) East Wall, midway down: A sign indicating the location of a fire extinguisher was posted, but no fire extinguisher was in the designated location. b) North Wall, near restroom: A large portable fire extinguisher was left on the ground, unmounted.

29 CFR 1910.157(e)(3): The employer shall assure that portable fire extinguishers are subjected to an annual maintenance check. Stored pressure extinguishers do not require an internal examination. The employer shall record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less. The record shall be available to the Assistant Secretary upon request. Welding Shop: On/before February 12, 2024, Class A fire extinguishers were not subjected to annual maintenance checks. Employees were exposed to smoke and burn related injuries from the potential fire hazard.
