29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Apartment building #300, Roof: On 2/15/2024, the employer exposed employees to fall hazards of approximately 32-feet to the lower ground level, in that a fall protection system was not provided to employees while repairing a roof leak.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Apartment building #300; Roof: On 2/15/2024, the employer exposed employees to fall hazards, in that the extension ladder side rails were not extended at least 3-feet above the eave of the roof to give employees a safe means of access/egress on and off the roof.
