29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   At the job site located at 4032 SW Huntoon St., Topeka, Kansas, 66604. employees engaged in construction activities working at ground level were exposed to struck-by hazards from falling tools or materials in that hard hats were not provided.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the job site located at 4032 SW Huntoon St., Topeka, Kansas, 66604, employees engaged in construction activities, working on building a new facade were exposed to fall hazards of approximately 12 feet to the concrete below in that fall protection was not utilized.
