The employer did not ensure that no opening in a stair rail system exceeds 19 inches (48 cm) at its least dimension. a) Knoxville Post Office: On or about 02-15-2024 and at times prior, the stairs used to gain access to the top of the mail sorting machine had a top rail but was missing the mid-rail on the open side of the stairs creating a fall hazard.
