On or about February 15, 2024, at a worksite located at 3415 Calumet Avenue in Manitowoc, Wisconsin; the employer did not provide for frequent and regular inspections of the job site to identify subcontractor employees exposed to the health hazards of crystalline silica generated by dry sweeping, and electrical shock hazards created by missing outlet covers, missing ground pins in flexible cords, and flexible cords without strain relief.

29 CFR 1926.405(b)(2): Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear. On or about February 14, 2024, to February 15, 2024, at a worksite located at 3415 Calumet Avenue in Manitowoc, Wisconsin; employees of Concrete Finishers Inc. were exposed to electrical shock hazards while using an outlet without a cover to power a concrete joint saw.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. On or about February 14, 2024, to February 15, 2024, at a worksite located at 3415 Calumet Avenue in Manitowoc, Wisconsin; employees of Concrete Finishers Inc. were exposed to electrical shock hazards while using a Ruwac heavy duty industrial vacuum cleaner that was plugged into a flexible extension cord. The extension cord was not provided strain relief and the outer protective sheath was disconnected from the plug.

On or about February 15, 2024, at a job site located at 3415 Calumet Avenue in Manitowoc, Wisconsin; the controlling employer did not prohibit employees of Concrete Finishers Inc. from dry sweeping where such activity could contribute to employee exposure to respirable crystalline silica.
