29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): On or about January 18, 2024, at 322 South Cucumber Street, Jefferson, OH 44047, the employer failed to ensure safety signage was in place to instruct employees to keep out of a restricted area on the Tsune high speed cutting saw line. An employee was injured upon accessing the area on the floor to move steel bars from the staging table to the feed table.

29 CFR 1910.179(b)(6)(ii): Where passageways or walkways were provided, obstructions were so placed that the safety of personnel was jeopardized by movements of crane(s): On or about January 18, 2024, at 322 South Cucumber Street, Jefferson, OH 44047, the employer failed to ensure employees utilizing the 15 ton P&H overhead crane in the saw department between saw 9 and saw 10 were protected from being struck by the load in that, an employee was struck while preparing to move steel bars in the area. The employee was pinned by the under the hook lifting magnet and steel container tubs located in the aisleway.
