29 CFR  1926.1203(e)(2)(iii): Before an employee entered the space, the internal atmosphere was not tested, with a calibrated direct-reading instrument, for oxygen content, flammable gases and vapors, and for potential toxic air contaminants, in that order. Any employee who enters the space, or that employee's authorized representative, must be provided an opportunity to observe the pre-entry testing required by this paragraph.  a) Jobsite - Manhole # 1, manhole # 2 and manhole # 3: On or about 2/15/2024, the employer allowed workers to enter multiple permit-required confined space locations without performing atmospheric testing for flammable gases and vapors, and for potential toxic air contaminants, before entering these areas.

Periodic monitoring was not used during the course of entry operations either.  a) Jobsite - Manhole # 1, manhole # 2 and manhole # 3: On or about 2/15/2024, the employer did not continuously monitor for atmospheric hazardous conditions and allowed workers to enter multiple confined spaces with only the initial reading for the first entry.
